Description
The Human Services Specialist in the Disabilities and Special Needs (DSN) Department is responsible for delivering personalized support to individuals with developmental disabilities and special needs. This role includes evaluating client needs, developing and managing care plans, and ensuring that clients can access essential resources and services. The Specialist collaborates with clients, their families, and service providers to foster independence, enhance well-being, and improve the quality of life for individuals with disabilities. Due to the dynamic nature of this role, duties can frequently change, requiring flexibility and a proactive approach to adapting as needed.
Examples of Duties

- Conducts comprehensive assessments with a team to determine the needs, strengths, and goals of clients. Develop and implement person-centered service plans that address each client's unique circumstances and aspirations.
- Coordinates and manages the delivery of services, including healthcare, therapy, vocational training, employment, and other support services. Ensure that clients receive appropriate and timely care.
- Maintains detailed and accurate case files for each assigned client, documenting all interactions, service plans, and progress. Regularly review and update service plans to reflect changes in clients' needs and circumstances.
- Advocates for the rights and needs of individuals with disabilities. Assist clients and their families in navigating complex service systems and accessing necessary resources.
- Provides support and guidance to families and caregivers, offering resources, education, and referrals as needed to enhance the care and support they provide.
- Responds to crises and emergencies, providing immediate support and coordinating appropriate interventions to ensure client safety and well-being.
- Builds and maintains relationships with community organizations, service providers, and stakeholders. Participate in community outreach efforts to promote awareness of DSN services and resources while collaborating with multiple agencies.
- Ensures compliance with all relevant policies, regulations, and accreditation standards. Prepare and submit required documentation and reports in a timely manner.
- Stays informed of best practices and emerging trends in the field of disabilities and special needs. Participate in ongoing training and professional development opportunities.
- Organizes and participates in outings and activities.
- Maintains and updates system related information, records, and data entries on the program's attendees.
- Facilitates compliance with state and county guidelines, standards and licensing requirements.
- Assists transportation coordinator with inspecting and maintaining cleanliness of vehicles being utilized for daily operations. Reporting any conditions requiring services to maintain safety.
- Completes performance evaluations for assigned, supervised employees and assists in developing effective goals and performance plans for the employee.
- Supports supervised employees in their ongoing professional development and ensures they stay up to date with the latest practices, training, and advancements in the field of Developmental Disabilities.
- Develops and monitors employee work schedule for assigned contracts, routes, special events and classrooms.
- Ensure the health, safety, and well-being of adults with developmental disabilities. Responsibilities include, but are not limited to, transporting, feeding, hygiene care, including toileting, lifting, transferring, administering medication, assisting with daily activities, and promoting the development of daily life skills.

All applicants tentatively selected for a position with Beaufort County Council will be required to submit to urinalysis to screen for the illegal use of drugs and to a background check prior to appointment.
