Position Summary
The HOPWA Housing Case Manager provides direct services to people living with HIV (PLWH) and their families with respect to addressing housing barriers and linkage to support services. Duties include: housing assessments on new clients, housing reassessments, identifying problems and needs, and working with the clients to meet those needs. Works closely with housing service providers within the community to maximize use of resources and develop partnerships, advocate for the client, and make referrals to other providers as needed. The HOPWA Housing Case Manager provides assistance for short-term rent payments, mortgage payment, and utility payments to help prevent persons from becoming homeless. The HOPWA Housing Case Manager is also responsible for maintaining accurate financial assistance records and service delivery records in addition to fulfilling evaluation and reporting requirements. Serves on quality management sub-committee as needed.

Essential Duties And Responsibilities
- Responsible for the management of a client caseload of up to 70 HIV persons:
- Coordinate HOPWA programs for Beaufort, Jasper, Hampton, and Colleton counties.
- Conduct assessments of all HOPWA clients requesting Short Term Rent, Mortgage, and Utility (STRMU), Permanent Housing Placement (PHP), or Tenant-Based Rental Assistance (TBRA) assistance and process requests for housing assistance.
- Completes HOPWA/TBRA applications with clients and obtains appropriate documentation for eligibility and recertification.
- Completes progress logs, action plans, and other Provide Software documentation in a timely & accurate manner.
- Responsible for the maintenance and overall organization of the client’s record (paper chart and electronic record).
- Responsible for ensuring programmatic accuracy of data collection and documentation; this includes data entry into the Provide Enterprise database and ensures that data is accurate and ready for submission; also responsible for HOPWA program monitoring and reporting for Beaufort, Jasper, Hampton, and Colleton counties.
- Conduct field visits to assess and support client and/or identify unmet needs and/or institutional visits as applicable.
- Ensure HOPWA participants understand their tenancy/participation agreements, their rights and obligations as participants, and their RW client rights and responsibilities.
- Coordinate and participate in community events and meetings to network with other collaborative agencies and service providers to secure appropriate, quality services for HOPWA program participants.
- Conduct training sessions and presentations to clients, colleagues and other collaborative organizations to explain HOPWA program/services to clients and community partners ; Deliver briefings and reports to colleagues
- Travels to outlying counties to assist with providing outreach services and information on HOPWA services to clients, other consumers, and external agencies.
- Liaison with community and network partnering agencies.
- Provide waived rapid testing to high risk negatives or individuals of unknown sero-status in testing initiatives and risk reduction strategies.
- Provides advocacy, referrals to and coordination with services and programs to ensure continuity of care as needed.
- Attends necessary trainings/meetings as required
- Performs other related duties as assigned by management.
- Minimum requirement of a Bachelor’s degree from an accredited college or university in a human services field related to Social Work, Health Education, Counseling or Public Health with at least three years of experience in case management/direct service provision in a human services setting.
- Master’s degree from an accredited college or university in a human services field related to Social Work, Health Education, Counseling, or Public Health with at least one year of case management in a human services setting working with PLWHA, preferred.
