Overview of Position: The Medical Case Manager is responsible for providing medical case management services for all assigned patient cases and the maintenance of those cases.
Qualifications: Master’s degree from an accredited college or university in a human services field related to Social Work, Health Education, Counseling, or Public Health with at least one year of case management in a human services setting working with PLWHA preferred. Minimum requirement of a Bachelor’s degree from an accredited college or university in a human services field related to Social Work or Counseling with at least three years of experience in case management/direct service provision in a human services setting working with PLWHA, preferably experience as a Ryan White Case Manager.
Essential Job Functions:
- Provides a range of patient-centered medical case management services to PLWH including access, linkage, and retention in HIV care and treatment services, coordination and follow-up of medical referrals, and ensuring timely and coordinated access to medically appropriate levels of treatment.
- Coordinates medication adherence with patients; Submitting ADAP applications and re-certifications.
- Provides medical case management services that result in a coordinated care plan, to include benefits counseling, support and information, including risk reduction and service literacy expansion.
- Completes all required assessments, bi-annual updates, and action plans.
- Provides accurate and complete documentation of activity through Progress Logs, Services Provided, Action Plan, Referrals, and other project-specific data and evaluation collection methods in Provide Enterprise (PE) and template assessments in eClinical Works (eCW).
- Assists the medical provider with the monitoring of indicator measures to ensure that they are met in the necessary time frame.
- Generates proper reports. Assists with data collection, reporting, and reviews for CQM.
- Attends necessary trainings/meetings as required. Completes Medical Case Management required trainings within 18 months of hire.
- Other duties as requested by the Program Director and/or Program Supervisor of Ryan White Services.
Knowledge/ Skills:
- Experience in the provision of health care services to people living with HIV/AIDS, both clinical and administrative.
- Working knowledge of the Ryan White CARE Act program requirements and HIV/AIDS Bureau performance indicators.
- Previous experience in a clinical outpatient setting, regarding the entry of patient data including but not limited to demographic, clinical, and other relative tracking information using programs such as eCW, Provide Enterprise, etc.
- Experience with treatment adherence and counseling among HIV positive patients, risk reduction counseling, and HIV testing.
- Strong attention to detail in addition to exemplifying the capacity to adhere to instructions and complete all assigned tasks in a timely manner
- Valid Driver’s License required.
- Ability to travel both in-state and to occasional national meetings.
