We are looking for a PT LPN (Every Other Weekend 11:00 pm - 7:30 am)

.RESPONSIBILITIES OF LPN, CHARGE NURSE:
- Provides direct and ongoing contact with the residents, physicians, staff members, families, and vendors
- Monitors the physical, mental, and social functioning of the resident
- Demonstrates teamwork ability
- Serves as a resident advocate
- Conducts thorough nursing assessments and incident reports of residents
- Follows standard nursing care practices for treatment and medication distribution
- Maintains current documentation and other clinical records, ensuring completeness in a timely manner
- Facilitates orientation and training of all necessary staff involved with the care of the residents
- Demonstrates willingness for continuing education
- Directs and oversees CNAs in day-to-day nursing care of residents
- Responds to IL emergency calls as needed
EDUCATION, SKILLS & ABILITIES:
- Must possess a current, unencumbered, active license to practice as an LPN in this state
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel
- Minimum 1-year nursing experience, preferable in a Personal Care or Assisted Living setting
