We are seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality care to individuals in their homes. As a valued member of our team, you will develop and implement care plans that prioritize client comfort, safety, and independence.
Your Key Responsibilities:
- Deliver skilled nursing care based on client Plans of Care and within scope of Massachusetts Nurse Practice Act.
- Collaborate with Registered Nurses to support and carry out care plans.
- Provide education and guidance to patients and families on condition management and at-home care.
- Monitor client health status, document accurately, and report updates to the clinical team.
- Utilize medical equipment safely and maintain infection control standards.
Essential Requirements:
- Active LPN licensure in Massachusetts.
- Nursing degree from an accredited institution.
- A minimum of 3 years' experience in home healthcare or medical-surgical settings.
- CPR/BLS certification.
- Reliable transportation and valid driver's license.
- Recent health clearance and TB test results.
- Fluent English language skills (spoken and written).
