About Our Clients:
We serve clients who are suffering from chronic and terminal illnesses due to toxic exposure. As an RN Case Manager, you will work closely with these clients and their families to develop personalized care plans that meet their unique needs.
Your goal will be to ensure clients receive the highest level of care and support in the comfort of their own homes. You will be responsible for conducting regular nursing visits to assess clients' health and well-being, managing medication, and coordinating care with healthcare providers.
Responsibilities:
- Conduct regular nursing visits to assess clients' health and well-being
- Develop and implement personalized care plans in collaboration with clients and caregivers
- Manage medication and coordinate care with healthcare providers
- Ensure home safety and prevent accidents or injuries
Requirements:
- Graduate of a state-approved school of professional registered nursing
- Bachelor's degree in Nursing (BSN) preferred
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two years of nursing experience, including one year in home care or a closely related field
