Senior Assessment Coordinator
We are seeking a skilled and experienced Senior Assessment Coordinator to join our team. As a Senior Assessment Coordinator, you will be responsible for overseeing the Resident Assessment Instrument (RAI) process and conducting assessments and care plan coordination for residents.
Key Responsibilities:
- Completes the MDS, CAA's and care plans within regulated time frames.
- Coordinates scheduling the RAI process with the interdisciplinary team.
- Assesses resident through physical assessment, interview and chart review.
