About the Role

As a Disease Intervention Specialist, you will work closely with individuals, families, and community organizations to prevent the spread of communicable diseases and substance use disorders.
Key Responsibilities
- Facilitate screenings and assessments to identify individuals at risk of substance use disorder or communicable disease transmission.
- Deliver education and outreach programs aimed at preventing drug overdose deaths and the spread of communicable diseases.
- BUILD and maintain partnerships with community resources, including Syringe Service Providers, mental health agencies, and treatment providers.
- Connect clients to a range of services, including community mental health, substance use treatment, harm reduction, primary health services, and recovery support services.
