Schedule Information
All Facilities work a Schedule of 36 / 48 for the two week pay period unless notated otherwise on job order
Orientation is bi-weekly
Start date on job is start date needed
Resource must reside 100 miles from facility to be considered
Assignment confirmation needs to be loaded within 24 hrs. of accepting offer
Unit Specific Information
Unit Exam specific to the contractor’s scope of practice within one year prior to start date; annual update required
Additional Details
Proof of 2 MMR vaccinations or a positive titer are required prior to start
Positive titer or proof of 3 Hep B vaccine series or declination can be accepted
The flu vaccine is required by 12 / 11 until our Flu requirement expires in March
Annual Physical or Physician note / statement within 1 year prior to start
Proof of 2 Varicella vaccinations or a positive titer are required prior to start
Valid 10 years from the administered date. Td will not be accepted. Declinations are acceptable
If positive TB record; Requires Proof of Chest X-Ray with in last 5 years to be accompanied by TB questionnaire
2 references required; 1 Charge or Higher; 1 must be supervisor reference
Background checks must be within 30 days prior to start
