Position Summary :
Provides skilled nursing care to patients in their Homes, Assisted Living Communities, Skilled Nursing Facilities, Memory Care Communities, Hospitals, and Independent Living Communities along various Healthcare Continuums as needed. Coordinate care with the patient, family, referring agency, and assumes responsibility for the coordination of care. Provides service in accordance with the Agency philosophy, standards and policies.
Essential Functions :
- Responds to a request for initial assessment within 24 hours
- Develops a written plan of care for each patient, assessing the physical, physiological, and social needs of the patient and documents
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management Educates the patient and family regarding pain management.
- Performs patient care responsibilities considering needs specific to the standard of care for patient’s
- Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical
- Provides skilled nursing care as needed to Covid-19 and non Covid-19 patients
- Obtains all necessary physician orders and collaborates with physician as patient’s needs
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical / health care
- Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems,
- Follows up with obtaining lab results and notifying physician of any significant
- Re-evaluates the need for continued care on an ongoing basis; initiates recertification as needed.
- Plans for discharge from the Agency with the patient and Makes referrals for alternative level of care if necessary.
- Promotes patient / family / caregiver education using various verbal and written communication techniques that take into account the patient’s / family’s cultural, ethnic and / or personal needs or preferences.
- Supervises Home Health Aides on an as needed basis.
- Initiates needed referrals to other disciplines and / or community
- Obtains needed supplies and equipment to provide
- Knowledgeable of current federal, state and agency regulations concerning patient

Graduate of an accredited school of nursing.
Current Nevada licensure as a Licensed Practical Nurse
Current CPR card and HIV certificate and Physical / TB free from communicable diseases statement Valid state driver’s license and reliable automobile.
Proof of current automobile insurance.
