We are looking for an exceptional Home Care Physical Therapist to join our team and make a real difference in the lives of those recovering from illness and injury.
About the Role
- Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
- Implement personalized treatment programs based on each patient's plan of care.
- Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
- Supervise and guide PT Assistants and PT Aides to ensure the highest quality of care.
Responsibilities
- Physical Therapy Assessment and Skilled Treatments.
- Client Care Coordination.
- Teaching, Supervision and Counseling Clients and Caregivers.
- Documentation of Interdisciplinary Communication Supervision of Ancillary staff.
- Problem Solving and Critical Thinking Skills.
- Documentation.
- Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
- Time Management Development of Plan of Care
Qualifications
- Proof Of Current/Valid Car Insurance
- Knowledge And Experience With Electronic Health Records
- Experience With Excel, Power Point, Word, Visio, Etc.
- Valid DL With No More Than 3 Moving Violations
- Ability to assist in patient lifts, transfers, and guarded functional activity training.
Credentials
- PT - Physical Therapist
- BLS - Basic Life Saving certification
Education
- Minimum required: Bachelor's degree
- Preferred/desired: Master's degree or doctorate
