Job Description
We are seeking a caring and compassionate licensed professional to join our team as a Clinical Social Worker. Reporting directly to the Clinical Director, this position plays an integral role in the provision of clinical outpatient services.
Key Responsibilities
Promote a welcoming positive customer service culture for clients, staff, and other stakeholders.
Maintain a balanced caseload with an emphasis on the development of environmental and psychosocial needs of clients.
Complete intakes and assessments to determine the appropriate level of care for new clients.
Ensure all provided services have preapproved insurance authorizations and meet necessary timelines.
Counsel individuals regarding psychological or emotional problems related to substance abuse and/or mental health.
Provide weekly group facilitation.
Provide individual counseling utilizing evidence based therapeutic interventions.
Develop and implement therapeutic treatment and aftercare plans using a client-centered approach.
Serve as primary therapist throughout changes in levels of care whenever possible.
Maintain client records within the electronic health record system including progress notes, clinical treatment plans, treatment plan reviews, discharge summaries, etc.
Complete all essential paperwork in a timely manner.
Coordinate with client and referral sources for discharge planning.
Coordinate with case managers and peer mentors for additional supportive services.
Interprets agency policies and procedures to assigned clients.
Monitors urine specimen activities required for substance abuse analysis.
Maintain coordination of care when needed between client and external agencies.
Attend weekly clinical case conference and supervision meetings.
Maintain confidentiality pertaining to clients and client records that meet 42 CFR and HIPAA regulations.
Acquire and maintain all CEU, Adult CPR/First Aid, supplemental training, etc. as required.
Qualifications
Master's degree in social work.
A license (LMSW or LLMSW) to practice social work in the State of Michigan.
Ability to obtain certification from the Michigan Certification Board for Addition Professionals upon hire.
Ability to conduct didactic and/or process groups.
Competency with Microsoft Office Suite.
Comfortable working with electronic health records.
Strong verbal and writing skills.
Valid driver’s license.
Ability to meet Federal, State, and local criminal clearance requirements.
