As an Access Navigator, you’ll play a critical role in providing compassionate, evidence-based support and referrals to individuals served within our integrated behavioral health framework.
What You’ll Do:
As an Access Navigator, you’ll serve as a key advocate for individuals navigating behavioral health services. You’ll help build connections, assess needs, and provide in-the-moment interventions to support individuals in achieving their wellness goals.
Key Responsibilities:
- Guide individuals through the Access Navigation workflow, collecting and utilizing Social Determinants of Health (SDOH) information to inform care.
- Build strong working relationships with individuals served and their support systems, maintaining consistent communication to ensure seamless access to services.
- Act as the primary point of contact for individuals served, ensuring follow-through and connection to care management teams.
- Collaborate with internal and external service providers to address individual needs in a timely and efficient manner.
- Serve as a knowledgeable resource on community services and programs, both within BHN and externally.
- Maintain accurate records and utilize electronic health systems to document interactions and progress.
- Support and mentor team members in implementing strategies to increase engagement and enrollment in care management programs.

What You’ll Bring:
- Bachelor’s Degree in a relevant field (required).
- Experience in behavioral health, case management, or community resources (preferred).
- Strong interpersonal and organizational skills with the ability to manage multiple tasks.
- Proficiency with electronic health records, telehealth platforms, and Microsoft Office tools.
- A collaborative, team-based approach to supporting individuals served.
