Join Our Team as a Nurse Care Manager at Behavioral Health Network!
Are you a compassionate and skilled nurse looking to make a real difference in people's lives? Behavioral Health Network is seeking dedicated professionals like you to join our team of Nurse Care Managers in the Behavioral Health Community Partners (BHCP) program.
In this role, you'll leverage your clinical expertise and passion for patient care to support individuals on their healthcare journey. As part of the BHCP team, you'll make an impact by:
- Providing expert Nursing Care Management, using evidence-based clinical theories and principles to improve health outcomes for our clients.
- Collaborating with our care team to develop and implement person-centered Integrated Care Plans that reflect the unique needs and preferences of each individual.
- Offering medical consultation to Care Coordinators and team members, including conducting nursing assessments and planning interventions that facilitate positive health outcomes.
- Monitoring and adjusting care plans to ensure the best possible results and effective resource use.
- Ensuring seamless communication and collaboration with all members of the Integrated Care Team, including primary care providers, specialists, home service providers, and family caregivers.
- Offering follow-up services, health-related quality improvement planning, and medication reconciliation through phone or in-person visits.
At BHCP we pride ourselves on putting the individual at the heart of everything we do. We work with adults aged 21-64, supporting them through the complexities of the healthcare system and helping them overcome barriers to better health.
What We’re Looking For: We need a dynamic and experienced Nurse Care Manager who brings:
- A Bachelor’s Degree in a related field
- At least 2 years of experience in the human services field
- Registered Nurse (RN) licensure and certifications
- Strong organizational and communication skills
- A valid driver’s license and reliable vehicle
