Your experience, knowledge and great skills will support the Northern Hope Center with the following :
- Working with treatment team and individuals to ensure successful referral to and engagement in next level of care.
- Providing information, referrals, and follow-ups as specified in individuals' treatment plans.
- Facilitating treatment groups for person's served.
- Maintain consistent communication through documentation and verbal contact.
- Complete client documentation in a timely and thorough manner.
- Complete EIM / ESM data input.
- Recognize and reports program specific concerns.
- Attends and participates in staff meetings, in-service trainings, and mandatory trainings.
Who are we?
The Dorchester Family Center provides long-term residential family substance use treatment for women and their children of all ages, from newborns to teens. The diverse experienced team at the Dorchester Family Center provides a variety of services including individual and group counseling, therapy and psycho-educational groups, individualized treatments plans, and medical and mental health referrals, among others. Our goal is to provide a safe and sober living environment to allow successful family reunification and long-term family stabilization.
