Join our team of energic and C ompassionate Professionals as a Care Coordinator I in the ICC program at Behavioral Health Network!
The Bilingual Care Coordinator provides high quality, community-based Wraparound services to youth and families. Use your facilitation skills to engage with a multi-disciplinary team of professionals treating youth with behavioral health needs and their families to achieve stability and success in the community. Care Coordinators help families connect to community supports, navigate different systems (school, juvenile justice system, behavioral health systems etc.) and support through crisis interventions.
Who are we?
Intensive Care Coordination Program (ICC) :
The Bilingual Intensive Care Coordination (ICC) program at Behavioral Health Network is a voluntary outreach service that uses 'wraparound' principles to guide the work for youth with serious emotional or behavioral health needs. Wraparound is simply the coordination of multiple services via a tailored plan to address the needs of the youth.
Qualifications / Requirements
- Bachelor's Degree in a related field with one year experience with youth with Serious Emotional Disturbance OR Associate's Degree with 5 years' experience working with youth with SED.
- Must be CANS-MA certified.
- Must be Bilingual in ( Spanish)
- Must have knowledge of mental illness / SED as manifested in children and adolescents as well as knowledge of treatment techniques.
- Must have familiarity with internet, word processing, and email systems.
- Experience in navigating any of the child / family-serving systems (preferred).
- Knowledge of community resources (preferred).
- Experience facilitating meetings or groups (preferred)
