Service Navigator works under the direct supervision of the ADAM Project Coordinator. The Service Navigator serves as the central hub of services and referrals for clients and treatment staff. The Service Navigator will fulfill assigned roles including but not limited to treatment team, intake, follow-ups, community liaison, administrative support, HIV counselor, and case manager.
Major Tasks, Duties and Responsibilities
• Conducts intakes, screenings and assessments for The ADAM Project.
• Maintains client files in accordance with all applicable federal, state, local, agency laws, and protocols.
• Collects and analyzes client demographic data and provide monthly reports to treatment and frontline staff.
• Monitors documentation of services provided in accordance with funder requirements and company protocols.
• Attends all required funder, agency, and departmental meetings/trainings.
• Provide administrative support to frontline and executive staff.
• Monitor and input GPRA data including intakes, follow-ups, and discharges.
• Ensure all GPRA intake interviews, discharges, and follow-ups are completed and entered within timeframes.
• Work collaboratively with Substance Abuse Counselors, Mental Health Counselors, and Outreach staff to refer clients to appropriate resources and services.
• Maintain professional relationships and act as liaison between ADAM Project and outside community organizations.
• Facilitate medical detox intakes for clients.
• Maintain a continuous calendar of outreach events.
• Delegate work to outreach staff including data entry, spreadsheets, social media activity, and community event appearances.
• Maintain and update a working list of community referrals for client services.
• Maintain contact with active clients throughout project and provide referrals and case management as needed.
• Provide routine pre/post HIV test counseling to clients.
• Complete linkage to care for HIV positive clients.
• Knowledge of diverse communities; including but not limited to: Gay/Lesbian/Bi/Trans/Questioning/Queer individuals, youth, communities of color, individuals experiencing homelessness, individuals involved in street economics, sex workers, and/or people living with HIV/AIDS.
• Knowledge of motivational interviewing and counseling.
• Serves as BHS inc. and ADAM Project representative at resource fairs, stakeholder meetings, and special events throughout the community.
• Serves as liaison between ADAM project and The Center and coordinates room scheduling and scheduling for events and meetings.

Prerequisite Qualifications
Following are the qualifications required to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act.
Bilingual abilities may be required within certain BHS programs.
A bachelor’s degree in a social service related field, or 2 years’ experience in providing services similar to those contained within this job description. Experience working with diverse racial/ethnic populations and LGBT individuals. Knowledge of HIV/AIDS risk behaviors and local service community, motivational interviewing techniques, and substance abuse service milieu.
Must have valid California driver’s license and liability insurance if driving personal vehicle on BHS business.
Vision, hearing, manual dexterity and eye-hand coordination must be adequate for performance of job duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff.

Qualifications
Salary : $20 - $30
