Your role at BHSN:
To work collaboratively with the team to provide Northwoods BHSN ESSHI tenants with a wide range of supportive services to assist tenants in maintaining independent living in permanent, supportive mixed-housing setting. All shifts are “stay awake” shifts.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Independently manage a caseload of Northwood tenants to ensure services are provided timely and accurately documented.
- Provide ongoing collaboration with the treatment team including internal and external support systems within the community to identify and coordinate needed resources to ensure resident needs are met including but not limited to benefit counseling, coordination of mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning
- Provide rehabilitative tenancy support services, both in the office and in the tenant’s home, as outlined in OMH Supportive Housing Guidelines, and document all services accurately and timely to all Northwoods ESSHI tenants
- Possibly facilitate staff meetings in the Supervisor’s absence
- Submit receipts, check requests, and purchase order requests into Papersave
- Track and submit data monthly for quality assurance purposes
- Assist with on-boarding/training new team members
- Assist with screening/admitting and discharging program participants
- Coordinate and facilitate regular on-site or off-site activities for residents to promote well-being/stability/sense of community
- Clerical – data entering, filing, etc.
- Report facility/client maintenance concerns to the Northwoods Property Manager to ensure a safe living environment for program participants and act as a liaison between the tenant and the landlord to resolve housing issues quickly
- Completes housing assessment and assists participants in developing individualized Housing Support Plans
- Complete inspections of units to ensure the dwelling is safe and meets Housing Quality Standards (HQS)
- Respond immediately to crises and offer intervention support in conjunction with BHSN Crisis and/or treatment team providers
- Complete required trainings as assigned
- Complies with BHSN and regulatory policies and procedures
- Participate in staff meetings, as scheduled
- Report on-site to other BHSN residential programs for support services and staff coverage as needed
SKILLS AND QUALIFICATIONS:
- Associate degree in human services-related field required, OR
- High School diploma and 3 years experience in the human services field required
- Experience working with special needs populations. Experience working with individuals recovering from severe mental illness, substance use disorders, and/or homelessness preferred
- Excellent interpersonal, engagement, communication, time management, and organizational skills required
- Ability to work independently and collaboratively
- Good data entry skills and proficiency in Microsoft Office software
- Ability to climb stairs; ability to lift up to 20 lbs
- Participate in lifting and moving resident’s belongings
- Valid NYS driver’s license, acceptable to BHSN insurance carrier, for required local travel
- Reliable transportation to travel locally
- CPR certification is required within 120 days of employment
- Ability to perform various tasks to ensure safe/sanitary dwelling such as cleaning, changing light bulbs, washing dishes, removing garbage, light shoveling, sanding/salting outdoor steps/walkways, etc.
