Some Licensed Practical Nurse (evenings and weekends) daily tasks may include:
- Supervising distribution of medications according to protocol
- Coordinating services with primary care providers and chemical dependency treatment for residents
- Developing and documenting service plans
- Ensuring that a safe dwelling is maintained
- Assisting residents with case management needs, including but not limited to transportation, linkage to other services and discharge planning
Your skills and qualifications:
- Valid NYS Educational Department CASAC-T certification OR valid NYS Licensed Practical Nurse
- Experience in or knowledge of alcohol/substance abuse delivery system preferred
- Valid NYS driver license and reliable transportation for required local travel
- CPR certification within 60 days of employment
- Must be able to climb stairs, bend, and lift up to 25 pounds
- Proficient in Microsoft Office software
