The Long-Term Care Coordinator is responsible for addressing systems-level challenges in service delivery for older adults with behavioral health disorders, and for discharge facilitation, aftercare consultation, and technical assistance for adult and geriatric individuals with serious mental illness who have been discharged from state psychiatric hospitals to less restrictive environments (community programs, nursing homes, and family placements) in Baltimore City.  The goal of this position is to facilitate successful transition from institutional care to community living and maximize community tenure and successful community living for those served.

This position reports to the Director, Special Populations and is a full time position working in a hybrid work environment in Baltimore, MD. BHSB will consider for employment qualified applicants with arrest and conviction records.

Essential Functions

  • Facilitate discharge planning with state psychiatric hospitals and local providers for consumers from Baltimore City and those who will live in the city.
  • Provide case consultation, technical assistance, provider trainings, and crisis response support to:
  • Community residential settings (including but not limited to families, residential rehabilitation programs, assisted living facilities, nursing homes, and shelters) Baltimore City Senior Centers and
  • Day programs (including but not limited to psychiatric rehabilitation programs and adult medical day care).
  • Identify and address system-level challenges in service delivery for older adults with behavioral health conditions by:
  • Assessing and developing community-based resources and
  • Providing strategic direction, data, and policy recommendations on a variety of initiatives.
  • Participate in case conferencing and problem-solving complex consumer situations and system-level challenges with the Behavioral Health Administration, community, inpatient and long-term care providers, Local Behavioral Health Authority (LBHA)s, Adult Evaluation and Review Services (AERS), Adult Protective Service (APS), and Community Forensic Aftercare Program (CFAP).  This includes facilitation of reconsideration of consumers who have been rejected by facilities and follow-up support of consumers placed in the community.
  • Collaborate with the State and Local Heath Departments and other stakeholders to address system challenges in services for older adults.

  • Work effectively and collaboratively with others, both internally and externally to BHSB.

  • Serve as part of BHSB’s disaster response team as needed, in any capacity that is needed.
  • To perform assigned work functions, this position and those it supervises may have access to protected health information (PHI). It is expected that the employee will operate from a place of integrity and assume an ethical obligation to protect the security and confidentiality of all PHI in accordance with BHSB’s policies.
  • An offer of employment for this position at BHSB is contingent upon the results of a thorough background check, including criminal history, which will require prior authorization from the candidate. 
  • Perform other duties as assigned. 

Experience and Education

  • Master’s Degree
  • 5 years experience in inpatient and outpatient psychiatric care with adult and geriatric consumers with Serious Mental Illness (SMI.)
  • Licensed Social Worker; Registered Nurse will be considered.
  • Advanced psychiatric skills in assessment, treatment planning, and evaluation of outcome-based system measures.
  • Professional communication and collaboration skills and ability to form strong working relationships across disciplines, settings, and situations.
  • Expertise in clinical training within a variety of disciplines, levels of preparation, and settings.
  • Responsiveness & initiative to facilitate system integration and expansion efforts.
  • Written and oral communication skills and database management.
  • Intermediate to advanced technical and analytical skills in word processing, spreadsheet, and other standard business software.

Supervisory Requirements: This position has no supervisory responsibilities.

This role may require travel to other locations for meetings and other business-related needs.  Ability to travel locally 100% of the time is required. It is important to have access to some form of transportation to travel to off-site meetings, conferences, and site visits.

This means that starting salaries are based on the requirements of each job and are non-negotiable, that the organization is not favoring people who are comfortable asking for a salary/hourly rate increase. The starting hourly/ annualized salary rate for this position is $79,725.

The benefits plan includes comprehensive Medical and HRA, Dental and Vision coverage, as well as a choice of 4 FSA plans and an employee paid Legal benefit. BHSB offers employer paid life insurance, short-term and long-term disability as well as a 403(b) plan with employer contribution, generous PTO and 13 days paid holiday. Medical plan participants receive a $5,000/$10,000 debit card to cover eligible deductible and copay expenses, depending on chosen coverage.
