General Description

The purpose of this position is to assess community patients that need assistance with primary care, health and self-management programs, chronic disease management, and improved access to mental health services. Provides strong case management skills to provide quality assistance according to the client's needs.

This position works under close to general supervision according to set procedures but determines how or when to complete tasks.

Duties and Responsibilities

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

  • Identifies patient needs and assists patient connect with community and/or primary care resources as part of a collaboration between government and private health care resources.
  • Identifies environmental impediments to client's progress through interviews and reviews of patient records.
  • Processes IHCP applications according to state policy and procedures.
  • Interviews customers for IHCP services and compiles data necessary to determine eligibility for specific assistance, providing necessary eligibility forms in a timely manner.
  • Gathers information in a thorough manner from all resources to fully document current patient statistics, treatment plan, and prognosis.
  • Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan.
  • Demonstrates recognition of patient's rights to privacy and confidentiality as well as understanding of the record needs of the organization.
  • Performs monthly follow-up care and intense case management based on severity of client's situation.
  • Collaborates with other professional to evaluate patient's medical or physical condition and to assess client needs.
  • Performs related work as required.

Minimum Education and Experience Requirements:

Requires a Bachelor's Degree in Social Services, Human Services or related field, supplemented by two (2) years of progressively responsible experience in social services casework or related work; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities.


Physical Demands:

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

Special Certifications and Licenses:

  • Must possess and maintain a valid state driver's license with an acceptable driving history.
  • Additional training may be required based upon area assigned.
