Discharge Coordinator

Job Summary
The Discharge Coordinator plays a vital role in the healthcare setting, ensuring a smooth transition for patients entering and exiting care. This position requires a detail-oriented individual who can effectively manage patient coordinate discharge planning, and maintain accurate medical records. The ideal candidate will have a strong understanding of Point Click Care(PCC) and ALTCS applications.

Responsibilities

• Coordinate discharge planning by collaborating with healthcare providers to develop comprehensive discharge plans tailored to each patient’s needs.
• Maintain and update medical records in compliance with regulatory standards, ensuring confidentiality and accuracy.
• Work closely with interdisciplinary teams to facilitate effective communication regarding patient care and transitions.
• Monitor patient progress throughout their stay, addressing any barriers to discharge in a timely manner.
• Provide support to patients and their families during the admission and discharge processes, answering questions and providing resources as needed.

Skills

• Experience in behavioral health settings is preferred but not mandatory.
• Strong organizational skills with attention to detail for accurate documentation.
• Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
• Ability to work collaboratively within a team environment while managing multiple priorities.
• Familiarity with case management principles is an asset.

Job Type: Full-time

Pay: $45,000.00 - $50,000.00 per year
