Responsibilities  Social Worker :

  • Facilitate social services for residents, ensuring emotional and psychosocial well-being.
  • Collaborate with families, staff, and external resources to create and maintain individualized care plans.
  • Assist with admissions, discharges, and transitions of care to ensure a seamless experience for residents.
  • Act as an advocate for residents' rights and help resolve concerns or challenges.

    • Qualifications  Social Worker :

  • Master’s degree in Social Work
  • Licensed Social Worker (LSW) in the state of Ohio.
  • Previous experience in a long-term care or healthcare setting is preferred.
  • Strong communication, organizational, and interpersonal skills.
