Job Summary
The Primary Integration Support Specialist works as the main member of a two-person team assisting a unique, charismatic, intelligent, and active adult Autistic woman. This role provides primary care, facilitates daily routines and supports social and community engagement while the Secondary Integration Support Specialist is focused on situational awareness and safety.
Essential Duties and Responsibilities
- Facilitate and support self care as prescribed by her program management.
- Facilitate and support home care responsibilities as prescribed by her program management.
- Facilitate and support daily routines as prescribed by her program management.
- Facilitate and support connections with other staff, friends, family and community as approved by her program management.
- Facilitate and support volunteer opportunities and employment as scheduled by her program management.
- Meet her day to day nutrition, activity, health and medical needs in the exact accordance to the direction given by her program management.
- Be a safe and secure, known and trusted support.
- Be a reliable and flexible team member.
- Communicate respectfully to individual and all team members with a focus on fulfilling job requirements as they are communicated and assigned by her program management.
- Support schedule changes with a positive attitude in an effort to support greater access to relationships and opportunities (staff, friends, family and community.)
- Provide safe transportation in a vehicle that meets program requirements during shifts or coordinating the use of the individual's program car to ensure greater access to relationships and opportunities (staff, friends, family and community.)
- Support the individual socially, emotionally, behaviorally to develop and maintain relationships and opportunities (staff, friends, family and community.)
- Follow the instructions, directions, training and guidance prescribed by her program management.
- Demonstrate day to day problem solving in accordance with program guidelines, policies and procedures.
- Complete required shift tasks and fulfilling Primary Role responsibilities and documentation.
- Pre and post plan and communicate with Secondary Role.
- Understand the Primary Role and how/when to utilize Secondary Role support during the shift.
- Actively participate in team meetings and team discussions
- Support other team members
- Build and maintain respectful and positive relationships with other team members.
- Be open to new learning
- Respond to feedback and supervision by Program management.

Knowledge: Education -High School Diploma or equivalent
Working Conditions
Requires high levels of alertness and physical mobility during shifts.
Support may take place in home, community, or facility settings, including during transportation or outings.
The role is part of a team-based model, not a solo position.
Certifications and Licenses:
Valid driver’s license, vehicle registration, vehicle inspection and vehicle insurance all in good standing required.
