Job Overview
The Wellness Nurse plays a crucial role in promoting the health and well-being of patients through comprehensive nursing care. This position involves working collaboratively with healthcare teams to provide support in our assisted living and memory care neighborhoods. The Wellness Nurse will utilize their clinical skills to assess, plan, implement, document, and evaluate patient care while ensuring adherence to best practices and safety protocols.
Duties
- Conduct thorough assessments of patients’ health status, including vital signs and medical history.
- Provide direct patient care, including ADL and medication assistance as needed.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Administer medications and treatments following appropriate techniques.
- Monitor patients for changes in condition, reporting any concerns to the appropriate healthcare provider.
- Educate patients and their families about health management, disease prevention, and wellness strategies.
- Assist in behavioral health assessments and interventions when required.
- Participate in quality improvement initiatives within the healthcare setting.
- Maintain accurate documentation of patient care activities and outcomes.
Qualifications
- Valid nursing license in the state of practice.
- Experience in an assisted living or memory care neighborhood is preferred.
- Proficiency in EHR documentation is a plus.
- Strong understanding of assessment and care planning processes and protocols.
- Ability to lift patients safely and effectively as part of patient care duties.
- Excellent communication skills with a focus on patient education and support.
- Ability to work collaboratively within a healthcare team while demonstrating strong critical thinking skills.
