Community Health Workers – Job Information
Key information about the CHW positions:
- Community Health Workers (CHWs) are frontline public health workers who are trusted members of and/or have an unusually close understanding of the community served. This enables CHWs to address barriers to health and well-being, improve primary care engagement, and reduce avoidable hospitalizations. This is critical work to help fight the very health disparities that caused COVID-19 to disproportionately and unjustly affect so many of NYC’s communities.
- These positions have been opened up publicly and qualified individuals, including but not limited to Contact Tracers, Optum Monitors and Resource Navigators, and anyone with clinical/healthcare/public health experience are encouraged to apply.
- CHW is the functional title. The official corporate title for the positions is Community Liaison Worker, which is represented by DC37, Local 371.
- These are facility-based positions. However, key components of the work involve working with patients in the community and at their home.
- The positions involve working with an interdisciplinary team of clinical and non- clinical staff to support the patients in Primary Care Adults.
- Each position has a 35-hour work week, and may require some weekends.
- The positions are temporary, grant funded positions, and are expected to last for up to two years.
- New York City residency preferred
- Salary: $49,875.00
- Different spoken languages preferred like Spanish, Mandarin, etc.
