This position will :

• Review new admissions for any special needs or issues that need to be addressed
• Assist with patient discharges from a hospital, clinic, or nursing home setting to return home
• Coordinate the care of patients with complex needs that require multiple services over time
• Handle crisis situations by networking with healthcare workers
• Help patients understand financial and emotional resources available to them

Required :

• Bachelor's Degree in Social Work
• Wisconsin Social Work license
