Job Summary
The Hospice End of Life Doula is a non-medical professional who is trained to help those nearing the end of their lives with their physical, emotional, and spiritual needs. End of Life Doulas work with the dying and their families to ensure comfort and ease during what can be a difficult process. The End-of-Life Doula works in partnership with the Interdisciplinary Team and the patient’s individualized plan of care.
Job Duties and Responsibilities
- Follows an individualized Plan of Care established by the interdisciplinary team for each patient.
- Immediately reports any changes to the RN Case Manager and interdisciplinary team.
- Reports unusual conditions, observations and pertinent information to the nurse.
- Participates in interdisciplinary team meetings
- Participates in Performance Improvement activities as requested.
- Provides intervention and support as needed related to terminal illness and loss within Scope of Practice.
- Provides short-term and intensive counselling to patients and their families.
- Provides emotional and spiritual support and makes appropriate referrals to other team members
- Facilitates communication among patient, family and hospice team
- Other duties as assigned.
