Job Summary And Qualifications
The Rehab Coordinator is responsible for overseeing and coordinating the daily operations of the rehabilitation services within the hospital. This includes ensuring the efficient delivery of patient care, managing scheduling and documentation, and supporting the multidisciplinary rehabilitation team. The coordinator will collaborate with physicians, therapists, and other healthcare professionals to create and implement individualized care plans for patients. Additionally, the Rehab Coordinator ensures compliance with regulatory standards, facilitates communication between departments, and monitors the quality and effectiveness of rehabilitation services to support optimal patient recovery and outcomes

Job Summary And Qualifications
The Rehab Coordinator is responsible for overseeing and coordinating the daily operations of the rehabilitation services within the hospital. This includes ensuring the efficient delivery of patient care, managing scheduling and documentation, and supporting the multidisciplinary rehabilitation team. The coordinator will collaborate with physicians, therapists, and other healthcare professionals to create and implement individualized care plans for patients. Additionally, the Rehab Coordinator ensures compliance with regulatory standards, facilitates communication between departments, and monitors the quality and effectiveness of rehabilitation services to support optimal patient recovery and outcomes
Key Responsibilities:
- Coordinate patient care by scheduling assessments, treatments, and follow-up visits.
- Collaborate with physicians, therapists, and nursing staff to develop and update individualized treatment plans.
- Monitor patient progress and adjust rehabilitation plans as necessary.
- Ensure proper documentation and compliance with hospital policies and regulatory requirements.
- Provide education to patients and their families regarding rehabilitation goals, procedures, and post-discharge care.
- Manage the inventory and ordering of rehabilitation equipment and supplies.
- Address patient and family concerns, advocating for timely and effective care.
- Participate in quality improvement initiatives and continuous education to enhance the rehabilitation program.
- Ensure department is staffed to meet the needs of patient care demands.
- Graduate of an accredited school in clinical specialty
- Current Missouri licensure in clinical healthcare specialty or eligible for state license transfer required upon hire. (i.e. Physical Therapist, Occupational Therapist, or Speech Therapist. If PTA or COTA, must have 7 years of experience in the care and management of the inpatient rehabilitation patient).
- Ability to operate basic computer and data management software and knowledge of hospital operations, rehabilitation services, and compliance standards. Strong organizational, communication, and interpersonal skills.
- Minimum 2 years recent clinical care experience in a rehabilitation or healthcare setting, with a focus on patient coordination.
