Job Description:
We are seeking a skilled LPN Healthcare Coordinator to join our team. This role is responsible for providing healthcare coordination and direct intervention for designated individuals in our Supportive Living program.
Key Responsibilities:
- Provide healthcare and wellness coordination for individuals in accordance with state and federal regulations.
- Act as liaison and advocate for individuals' healthcare with medical and interdisciplinary teams.
- Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring.
- Implement and train employees on new and revised health-related policies and procedures.
- Participate in medical appointments and individual meetings as required.
- Manage transitions of care to and from hospitals, nursing homes, or other agencies.
- Maintain health-related records and complete documentation and assessments.
- Develop and update individual health risk plans to ensure appropriate medical care.
Requirements:
- CURRENT Ohio LPN license
- Experience in Community/Long-term care
- Experience working with individuals with IDD and/or mental illness
