- Relevant Position Duties
The Adult Day Professional most commonly accomplishes the following functions:
- Performance of Health-Related Activities if needed during Adult Day Support hours (assistance with self-administration of medications and/or treatments, implementing physician orders, etc.)
- Social & Community Support (support with new and existing relationships, accessing community services, activities and events)
- Skill Development & Goal/Outcome Support (taking steps to assist individuals in personal achievement and accomplishment)
- Assistance with Daily Living (preparations, personal care and hygiene tasks, day-to-day support and navigation)
- Performance of Health-Related Activities if needed during Adult Day Support hours (assistance with self-administration of medications and/or treatments, implementing physician orders, etc.)
- Person Centered Planning & Collaboration (implement all individual support and behavioral support plans, participate in care planning discussions and processes)
- Document Services Provided (maintain consistent documentation and written descriptions of services/support provided, mileage logs, shift start/stop times)
- Report and document all incidents impacting individual health and welfare
- Adhere to local, state and federal compliance requirements, rules and regulations
- Effectively take immediate actions to protect the health and welfare of the person(s) receiving services
- Participate in all trainings and recertifications as required
- Check and respond to any texts/emails/calls from Benchmark during each shift
