Resident Care Coordinator

As a Clinical Coordinator, you will partner with the Resident Care Director (RCD)/ Mind and Memory Director for all administrative tasks for the Wellness Department.    Duties/Responsibilities: Checks ADL books/resident electronic health record (myUnity) for documentation and completion of care. Checks paper/electronic Medication Administration Records (eMARs) and Medication Observation Records (eMORs) and verifies that medications have been given and treatments completed.  Reports any omission to the RCD for follow-up. Develops Resident Care Associate (RCA) staffing schedule in conjunction with the RCD. Responsibility to support and assist RCA staff with training and in-service completion Assembles binder/non-EHR Resident Records for new move ins. Updates resident face sheet information. Enters details of resident care needs into the Yardi/Resident Summary Template (myUnity). Ensures accuracy of RCA Assignments on paper or in Yardi and provides them to staff (if using Yardi/paper system). Assists the RCD with follow-up in obtaining Care Plan signatures. Orders and maintains sufficient supplies for the Resident Care department. Coordinates Quality Assurance and family meetings in conjunction with the RCD. Provides the Receptionist/Concierge with documentation (face sheets, DNRs, medication lists, etc.) for the Resident Emergency binders upon move-in, as required, and when changes occur.
