Position Details:
Weekday Per Diem (flexible with no minimum time requirements)
Setting: SNF, PC, IL, Outpatient
EMR: Net Health

Qualifications:
Active PA COTA License in good standing
CPR certification
Current PPD/TB test
Covid vaccination or approved exemption

Responsibilities Include:
1. Providing effective, personalized rehabilitative services
2. Recording patient responses to treatments and reports outcomes of treatment through proper documentation in Net Health and facility documentation systems
3. Establishing relationships with your patients to keep them engaged and positive about their treatment
4. Providing guidance to family members and facility staff
5. Conducting yourself in a kind, courteous and professional manner to all patients, family members, facility staff and fellow team members
6. Keeping a positive, teamwork approach
