Key Responsibilities :
- Conduct thorough patient evaluations and assessments.
- Develop personalized treatment plans based on individual needs and goals.
- Implement therapeutic interventions and monitor progress.
- Educate patients and their families on treatment protocols and wellness strategies.
- Collaborate and facilitate with other healthcare professionals to ensure coordinated care.
- Maintain accurate patient records and documentation.
- Attend meetings and case conferences
- Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll
- Completion of Evaluation, Plan of Care, Reassessment, and Discharge visits and paperwork.
