The Licensed Practical Nurse (LPN) Team Lead role role has been developed to assist the Director of Nursing with coordination of care and to ensure the delegated tasks of care are completed. This role may be accountable for the supervision and quality of care delivered by those tasks delegated to the Team Lead by the Director of Nursing (DON) or RN Designee. At all times it is recognized that the Team Lead will engage the RN if the delegated task exceeds the knowledge, experience, and/or competency of the Team Lead in order to assure resident safety and quality of care.

- The LPN Team Lead participates in the development, implementation and modification of the nursing/interdisciplinary care plan with other involved healthcare providers.
- This role assigns nursing activities or tasks to other LPN and/or unlicensed personnel and oversees the completion of that activity or task.
- They are responsible for gathering and compiling data and reporting care issues identified or delegated to them.
- The LPN Team Lead conducts a focused observation of the resident's health status and reports appropriately.
- This role will also onboard new associates to the community and monitor staffing hours in collaboration with the Staffing Coordinator.
- The LPN Team Lead will assist the DON, or RN Designee, and participate in staff management as needed.
- LPN licensure in good standing with the State Licensing Board.
- 5 or more years of LPN experience in a Long Term Care setting, or equivalent.
- BLS certified.
- Must be able to read, write and communicate in English.
