Our Social Services Designee coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. You will act as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
Responsibilities
- The Social Services Designee coordinates the overall psychosocial care plan for residents.
- The Social Services Designee provides support and advocates for resident/patient and their family.
- The Social Services Designee will develop and maintain community resources for residents.
Qualifications
- Related coursework in social work or psychology
- Hospital or nursing home experience, preferred
- Able to read, write and communicate in English
