Position Summary:
- Responsible for educating clients on a variety of health issues and available community programs.
- Utilizing the Pathways HUB model, work as a liaison between health organizations and clients to provide relevant information and links to services and supports and guide individuals towards a better understanding of their health and opportunities for successful aging.
Essential Duties and Responsibilities
The following description is a general representation of the key duties and responsibilities of the position.
- Create connections with vulnerable older adults and healthcare providers to increase access and minimize barriers to care.
- Contact prospective clients within 48 hours of receiving a referral, and if needed make a minimum of 2 other contact attempts within the following 2 weeks and document in system.
- Verify eligibility, complete consent form and all checklists as needed to enroll clients in the program.
- Facilitate healthcare and social service navigation. Make best efforts to ensure clients schedule and attend medical appointments with any providers as needed.
- Accompany clients to medical appointments as needed.
- Work with enrolled clients to identify needs and connect them to community-based resources and services (i.e. transportation, housing, etc.).
- Educate enrolled clients on topics relevant to their health utilizing a best practice curriculum.
- Make best efforts to ensure all clients without a primary care provider are connected to a medical home within 30 days of program enrollment.
- Submit all required documentation and enter client data in a timely manner.
- Attend assigned monthly meetings and all scheduled mandatory training, and conference calls.
- Advocate for underserved individuals to receive appropriate services.
Qualifications:
- Specific knowledge, training or skills required to perform the duties of this position including specific courses, training programs or required certifications. The requirements listed below are representative of the knowledge, skills and abilities required for the position.
- High school diploma or GED.
- Completion of approved Community Health Worker training program.
- Current certification as a Community Health Worker through the Ohio Board of Nursing.
- Strong verbal, written, oral and electronic communication skills.
- Excellent problem solving, customer service and interpersonal skills.
- Ability to navigate the healthcare system to meet the client’s needs.
- Proven ability to work independently and self-motivated to succeed.
- Excellent organizational skills and ability to handle multiple tasks and prioritize work.
- Demonstrated proficiency in the use of computer software (e.g. Microsoft Outlook and Office).
- Knowledge and understanding of the needs of persons with chronic health conditions and their family/friend caregivers.
- Familiarity and adherence to all ethical standards, laws, and regulations pertaining to confidentiality, personal health information, and adult mistreatment.
- Demonstrated sensitivity to ethnic and cultural diversity and age-specific challenges of persons with chronic health conditions and their family/friend caregivers.
- Able to deliver service and document all activities in the computerized record management system.
- Must possess a valid Ohio driver’s license and appropriate vehicle insurance.
- Must pass a BCI and FBI criminal background check.
B. Previous Experience that is necessary to qualify for this position:
- Prior experience working in a healthcare setting preferred.

This job operates in a professional office or home office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- While performing the duties of this job, the employee is regularly required to stand, sit; talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, keyboard and telephone.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Able to work remotely if necessary.
