Role Description
This is a full-time on-site role for an Intake Coordinator located in San Marcos, TX. The Intake Coordinator will be responsible for managing communication, providing exceptional customer service, coordinating cases, utilizing medical terminology, and processing insurance information.
Key Responsibilities:
Intake Process Management
- Receive referrals from caseworkers and initiate the intake process promptly.
- Collect and review patient information, including medical history, diagnosis, and insurance coverage, ensuring accuracy and completeness.
- Verify insurance benefits and coordinate with the billing department for proper billing procedures.
- Communicate with referring caseworkers and clients to gather additional information as needed.
Communication and Coordination
- Act as the primary point of contact for patients & families throughout the intake process.
- Liaise with case managers to facilitate a smooth transition from referral to admission.
- Provide clear and concise information about our services to patients and families, addressing any questions or concerns.
Documentation and Record-Keeping
- Maintain accurate and up-to-date electronic records of all patient intake activities, ensuring compliance with regulatory requirements and company policies.
- Complete required forms, contracts, and consent documents for each patient, ensuring proper documentation.
- Manage and track incoming referrals using spreadsheets and software tools, ensuring timely updates.
Client and Care Team Coordination
- Serve as the main point of contact for new clients, building positive relationships and addressing initial inquiries.
- Coordinate with caseworkers and healthcare professionals to ensure comprehensive care plans are in place.
- Maintain up-to-date caregiver policies and share relevant information with team members as needed.
- Monitor utilization reports weekly and address unused hours by coordinating with caregivers and clients.
- Assist in preparing and submitting documentation for new client intake.
- Collaborate with the care team to ensure all client needs are met efficiently and effectively.
Qualifications
- Knowledge of HHA Exchange or similar EMR systems
- High school diploma or equivalent required; Bachelor's degree in Healthcare Administration or a related field preferred.
- Knowledge of Medical Terminology
- Understanding of Insurance processes
- Excellent organizational and interpersonal skills
- Ability to work effectively in a team environment
- Previous experience in healthcare or home care industry is a plus
