SUMMARY:
The Quality & Care Coordinator plays a pivotal role in patient care coordination, ensuring that patients receive comprehensive and continuous healthcare services. Key responsibilities include care coordination, community outreach, and implementing quality improvement initiatives. This role involves close collaboration with healthcare providers and community resources to improve patient outcomes and enhance the overall patient experience. Additionally, the Quality & Care Coordinator will serve as a medical assistant to the healthcare provider based on the needs of the clinic.
ESSENTIAL FUNCTIONS
- : Monitor individualized care plans and connect patients with community resources, financial assistance programs, and support services. Ensure continuity of care by collaborating with physicians, specialists, and other healthcare providers. Empower patients to take an active role in their healthcare.
- : Conduct regular phone calls to monitor patient progress and coordinate follow-up for any new or ongoing health issues.
- : Facilitate smooth transitions of care between different healthcare settings (e.g., hospital to home, specialist referrals). Ensure that all necessary information is communicated effectively to all parties involved.
- : Participate in quality improvement initiatives aimed at enhancing patient care, safety, and satisfaction. Find opportunities for improvement and contribute to the development and implementation of best practices. Assist healthcare providers in closing quality care gaps identified using specialized reports
- Perform duties within the scope of a Medical Assistant, such as scheduling appointments, communication of information from providers to patients, room cleaning and preparation, escorting patients to exam rooms, obtaining vital signs, and documentation of care in the patient encounter. Under the order of a Healthcare Provider, obtain specimens, draw blood, perform point of care tests, give intramuscular and subcutaneous injections, preform electrocardiograms and ear lavages, remove sutures or staples, and perform dressing changes.
- Serve and protect the practice by adhering to Benson Hospital organizational and professional standards, values, policies and procedures, federal, state, and local requirements.
- Perform related duties as assigned while upholding the highest internal and external customer service practices.
MINIMUM QUALIFICATIONS
EDUCATION: High school diploma or equivalent; completion of a Medical Assistant training program preferred.
EXPERIENCE: Minimum of 1 year of clinical experience. Experience in a rural healthcare setting is preferred.
LICENSURE OR CERTIFICATION:
Certified Medical Assistant (CMA), Basic Life Support (BLS) certification. Exceptions made for candidates who meet Requirements of Article 4 of Arizona Administrative Code, Chapter 16, Arizona Medical Board (R4-16-401).
KNOWLEDGE, SKILLS, AND ABILITIES:
· Knowledge of general patient care practice, methods, and regulations.
· Skill in assisting with more complex patient care programs.
· Ability to read and interpret documents such as quality reports, patient result reports, operating and maintenance instructions, and procedure manuals.
· Ability to complete routine reports and correspondence.
· Ability to listen and accurately interpret others’ communication or instructions to take appropriate action.
· Ability to communicate with positivity and effectiveness with patients, staff members, and with the public.
· Excellent communication and interpersonal skills.
· Ability to work independently and as part of a multidisciplinary team.
· Proficiency in electronic health records (EHR) and other healthcare technology.
· Strong organizational and time management skills.
