What are the responsibilities and job description for the Perinatal & Family Health Program Specialist position at Benton County (OR)?
JOB SUMMARY
The Perinatal & Family Health Program Specialist works with a team of nurses to assist in coordinating referral and enrollment in nurse home visiting programs including Babies First!, Family Connects Oregon, and CaCoon. The program specialist is responsible for connecting with active/potential referral sources, establishing/strengthening referral pathways, scheduling home, office, or telehealth visits for people and families in the hospital or clinic, fielding public information lines to answer questions and reschedule visits, conducting post visit follow-up phone calls with each client for the purpose of monitoring client satisfaction, tracking referral outcomes, and attending virtual/in person community outreach events. In addition, a portion of FTE for this position is dedicated to perform the work of community alignment specialist for the Family Connects Oregon program.

- This position has the potential for two salary raises within your first year. Increases are given at the end of the 6 month probationary period and at the 1 year anniversary, as long as your performance is in good standing! Increases are given until the maximum of the salary range is reached.

The following minimum qualifications are REQUIRED for this PS21 position:
- 1 year of education or technical training beyond High School in Public Health, Education, Community Health, Health Services; AND
- 1 year of work experience in Public Health, Education, Community Outreach, Health Services, or Health Navigation.
- Associate's or Bachelor's Degree in Public Health, Education, Community Health or related field; AND
- 6 months of work experience in Public Health, Education, Community Outreach, Health Services, or Health Navigation.

Preferred Knowledge, Skills, and Abilities
- Bilingual Spanish-English highly desired.
- Experience with culturally diverse populations and the perinatal, maternal, child populations.
- Has knowledge and experience connecting with community and social services in Benton.
