The Position
GENERAL SUMMARY: The HIV Social Worker will provide case management services to individuals living with HIV/AIDS. The role focuses on helping clients coordinate their care and build community support to become independent and self-sufficient. Strong verbal and written communication, assessment, motivational interviewing, and intervention skills are required. The position involves integrating social work practices with the HIV Case Management Team's overall functioning.
ESSENTIAL JOB FUNCTIONS:
- Maintains a comprehensive understanding of the statewide standards of the HIV Medical Case Management and adheres to the requirements outlined in these standards.
- Develops and demonstrates knowledge of HIV/AIDS related conditions and treatment and uses knowledge to educate clients and people in their support systems.
- Provides education to clients on HIV, medication and medical appointment adherence, viral load and CD4 lab results, and risk reduction strategies.
- Networks with community providers and partnerships for improved service delivery and reduce HIV-related stigma.
- Assists with screening intakes for new clients.
- Completes, implements, monitors, reviews, and updates client's comprehensive assessments, individual service plans, and eligibility assessments renewals as required by the Department of Health (DOH).
- Accesses and coordinates medical, housing, and pharmaceutical services for clients by serving as an advocate and liaison between clients and community health/social services providers.
- Accesses and coordinates medical insurance/programs enrollment for clients. These may include but not limited to, Medicaid, Medicare (parts A - D), Individual Plans Inside the Exchange (QHP), Early Intervention Program (EIP), and Evergreen Health Insurance Program (EHIP).
- Provides clients with information and referrals to a wide range of community services.
- Develops and maintains positive working relationship with medical providers, housing resources, and other social service providers. Facilitates referrals for such services when necessary and appropriate.
- Serves as a motivated member of a small team, self-directed in performance and able to work both collaboratively and independently.
- Participates as a member or leader on various committees, task forces, and coalitions within the Health District, community, and State. Organizes and facilitates various support/educational groups for clients as indicated.
- Maintains professional, well written, comprehensive, and accurate client record, documenting interactions with clients and/or on behalf of clients and according to program standards (Provide database).
- Excellent written and verbal communication skills required, along with the demonstrated ability to work effectively under stressful conditions.
- Travels to visit clients and attend relevant meetings and trainings as necessary.
- Develops and maintains medical records in accordance with agency Policy and Procedures.
- Other related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES: This position requires the knowledge of case management principles, motivational interviewing and counseling techniques, and the ability to work with diverse clientele with various medical, psychosocial, substance abuse and mental health issues. Basic knowledge of how HIV is transmitted, acquired, and treated. Models and maintains appropriate professional boundaries at all times with all clients. Competence in Spanish, both oral and written is desired.
The Requirements & Selection Process
MINIMUM QUALIFICATIONS: Bachelor's Degree in social work or psychology, or other behavior health sciences and two years of social service experience. Master's Degree preferred.
ADDITIONAL REQUIREMENTS: Performance of job duties also requires driving on a regular basis, a valid Washington State driver's license, the use of the incumbent's personal motor vehicle when a District fleet vehicle is not available for use, and proof of appropriate auto insurance. This position is not a hybrid or remote position. This role requires onsite, in-person services.
