Position Overview :
The Director of Care Management has direct responsibility for all relevant program development, implementation, and monitoring in accordance with business, State and / or Federal guidelines. This position recommends, develops, and implements initiatives and evaluates outcomes related to care management performance. Works collaboratively with Wraparound partners for quality improvement.
Responsibilities :
- Assist COO in supervising business functions as it relates to service delivery
- Create relationship driven people initiatives
- Build trust and partnerships with interdepartmental subject matter experts
- Lead and manage the quality of in-person care management
- Works as a liaison with external organizations to ensure lines of communication are in place for member engagement
- Responsible for monitoring all work product to ensure it is in compliance with all State and / or Federal mandates
- Evaluates departmental operations with a focus on process improvement
- Continuously improving effectiveness and efficiency of processes and program
- Aligning services and care delivery to the strategic plan
- Additional duties as assigned
