Bergen's Promise is the Care Management Organization (CMO) of Bergen County as part of the New Jersey Children's System of Care. We are a rapidly growing, mission driven, non-profit social service agency. We are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).
Bergen's Promise serves youth up to age 21 and their families. The youth we serve are experiencing serious behavioral, emotional, mental health, substance use challenges and / or developmental disabilities.

Position Overview
The Social Worker Supervisor fully supports the development of care managers towards enhanced skills and job satisfaction as well as fully supporting methods and technology to support the quality care management and person-centered services for members while creating an ongoing culture of learning, knowledge, and retention within the Care Management Department.
Duties & Responsibilities
Follow the Wraparound model in all interaction with children and families.Oversee Care Manager's collaboration with family to organize and coordinate the development of a Child Family Team which shall consist of a minimum of the following members :The youth and parent / caregiver
Informal and natural supports the youth or family wishes to includeA representative from the FSO if desired by the familyA clinical staff member who is directly involved in the treatment of the youth, if desired by familyRepresentation from outside agencies the youth is involved with including but not limited to education, probation, DCPP or DDD workers as applicableCollaborate with the Family Service Organization (FSO) to initiate caregiver support.Ensure that all Child Family Team Meetings occur within the required time frames.Monitor quality assurance of service providers with regard to implementation of strategies and services as outlined in the Individual Service Plan, regulatory standards and State guidelines.Monitor ongoing quality assurance of assigned staff performance standards, complete required performance evaluations and implement applicable performance improvement plans.Oversee Care Manager's facilitation of families securing and maintaining appropriate Medicaid coverage within required time frames.Adhere to Medicaid standards and regulations governing targeted case management. Ensure that all direct reports do the same.Ensure that documentation is posted within the electronic record and filed in hard chart within required time frames and compliant with the applicable regulatory standards.Monitor Care Managers' completion of timely and accurate Family Crisis Plans.Review / Approve Individual Service Plans to confirm strength-based approach, include both formal and informal resources in addition to being complete, timely and accurate.Ensure care management team members comply with Agency required critical / unusual incident report protocol.Provide supervisory consultation to on-call staff for all Agency involved youth / families when scheduled; follow agency on-call protocol.Adhere to Agency Code of Ethics in addition to applicable professional boards and maintain appropriate professional demeanor and personal appearance. Requirements
What Qualifications & Skills are needed to apply?
Master's Degree in social work, counseling, psychology, or a related fieldLicense applicable to your degree (LSW, LCSW, LAC, LPC)Minimum one-year experience providing supervision to a team of direct reports.Experience with individuals (preferably children) with substance use disorders, and / or intellectual and developmental disabilities a plusBilingual language skills a plusExemplary time management and organization skillsCulturally competent verbal and written communication skillsBasic technology proficiencyValid driver's license required
