What are the responsibilities and job description for the Behavioral Health Consultant position at Berks Community Health Center?
Principal Duties and Responsibilities
- The ability to establish rapport quickly and to communicate effectively with patients, their family members , Berks Community Health Center staff, and other treatment team members as identified by patient. Using skills such active listening; communicating information in a jargon-free, non-judgmental manner, using terminology and adapting to the preferred modality of communication of the patients and families served.
- The ability to function effectively as a member of a professional team that includes behavioral health and primary care providers, patients, family members, among other treatment team providers.
- The ability to conduct brief, evidence based and developmentally appropriate screenings and to conduct or arrange for detailed assessments when indicated. This includes completing screenings for depression, anxiety, substance abuse, self-injurious behaviors, cognitive impairment, or any behavior that may compromise the patients overall health. And if/when required provide linkage to referral source for follow up evaluation and treatment.
- The ability to create a patient caseload and provide follow up brief interventions to those patients.
- The ability to use the electronic medical record to support and improve integrated healthcare. To ensure efficient and effective healthcare services documentation in the electronic medical record will be completed with 48 hours of encounter with patient.
- The ability to create and implement integrated care plans, ensuring linkage to an array of needed services, and exchange of information among patients, family members, providers, and other treatment team members as identified about such care plans and services. In doing so the BHC will identify the type and intensity of services required for the patients.
- The ability to safeguard patient privacy and confidentiality with respect to communication, documentation, and data.
- The ability to apply principles of population based care for preventive and chronic care conditions.
- The ability to be an active participant in individual/group supervision, case conferences, quality improvement meetings, staff/departmental meetings, and other scheduled activities as required.
- Refer patients prescribed psychiatric medications by BCHC’s medical provider to BCHC’s consulting psychiatrist for recommendations.
- The ability to return phone calls to patients, family members, among other treatment providers, seeking appointment and/or an area of concern.
- Actively identify and support linkage to community resources (professional services, educational, spiritual, vocational, self help groups, cultural, social, etc. that support the patients’ goals and interest.
- Other duties as required.
Knowledge, Skills, and Abilities
Key Competencies
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical mental health conditions.
- Ability to work through brief patient contacts as well as make quick and accurate clinical assessments of mental and behavioral health conditions and needs.
- Competent in providing cognitive and behavioral health interventions to patients with medical, behavioral, and/or substance issues in primary care setting.
- Should have knowledge base of primary healthcare pace and be comfortable working with interdisciplinary teams.
- Ability to manage multiple responsibilities and emergency situations successfully.
- Excellent verbal and written communication skills.
- Demonstrate a high level of problem solving skills to better serve patients and their team members.
- Strong attention to detail and accuracy.
- Ability to utilize computers for data charting and information retrieval.
Job Type: Full-time
Pay: $70,000.00 - $80,000.00 per year
