Position Summary
The Medical Assistant (MA) serves as member of a clinical support team that along with a Nursing Care Coordinator (NCC) and Patient Services Representative (PSR) support a designated Provider to integrate each patient’s care across all conditions and health care needs – both within BCHC and between BCHC and all other health care providers and settings. Under the leadership of the Nursing Care Coordinator (NCC) and the Provider, the MA carries out processes that operationalize BCHC’s mission to provide exceptional patient-centered primary and preventive healthcare for all residents of Berks County, regardless of economic status. The MA actively supports BCHC in its efforts to maintain designation as a Federally Qualified Health Center (FQHC) and as a Patient Centered Medical Home (PCMH). Participates actively in BCHC’s mission to provide exceptional patient-centered primary and preventive healthcare for all residents of Berks County, regardless of patient status.
Job Description
- ·Assumes primary responsibility for complete and accurate pre-visit preparation in accordance with Chart Prep Checklist.
- · Obtains all diagnostic results since last visit including but not limited to: Lab results; Diagnostic Imaging results; Verifies/validates screening test status appropriate to age and sex; Last mammography date and results; Last colon screening date and results; and PHQ-9 screening.
- · Determines any ED visits or inpatient admissions to local facilities
- · Obtains discharge summaries as appropriate.
- · Obtains last well child visit information.
- · As a key member of the patient care team, participates in all patient visits.
- · Updates pertinent patient history and significant interim issues.
- · Reviews current medications for reconciliation by provider.
- · Queries patient to determine whether they have had any visits to ED, inpatient admissions or visits to specialists that are not known to BCHC.
- · Provides patients with appropriate consent for treatment.
- · Completes patient self management goal sheets.
- · Assures patient is appropriately clothed in order to be, easily examined by the healthcare provider.
- · Communicates readiness of patient to examining healthcare provider and provides briefing of relevant and significant issues related to patient presentation.
- · Provides chaperonage to provider as required by organizational policy or specific request.
- · Arranges translator support to meet patient need as necessary.
- · Prints and/or prepares necessary forms for healthcare provider documentation.
- · Prints and provides patients with appropriate documents at conclusion of visits including but not limited to: Self-care management goal sheets; Clinical visit summaries; Patient education materials; Provider orders that cannot be e-prescribed or as requested by patient; and Documents concise, pertinent information and observations in the electronic medical record.
- · Documents test results in laboratory log book and in the electronic medical record.
- · Ensures orders for services provided in the electronic medical record are consistent with established procedures and protocols.
- · Performs direct patient care activities in accordance with BCHC policies and procedures and consistent with scope of practice.
- · Acquires and records vital signs, height, and weight measurements, performing other initial screens or tests which may be necessary.
- · Performs visual examinations and audiology screening as necessary, according to prescribed standards.
- · Administers child and adult vaccinations in accordance withstanding and provider orders.
- · Assists providers in exam rooms as necessary to perform procedures.
- · Performs point of care testing as necessary, such as urine dips, urine pregnancy tests, blood glucose testing and development of hemoccult slides.
- · In coordination with and under the direction of the NCC, documents tracking activities associated with Patient Centered Medical Home (PCMH).
- · In conjunction with Lab Technician and other care team members, participates in Lab Tracking activities in accordance with the BCHC Lab Tracking Policy and Procedure.
- · In conjunction with the designated Imaging Tracking Coordinator and other care team members, participates in Imaging Tracking activities in accordance with the BCHC Imaging Tracking Policy and Procedure.
- · Obtains necessary documentation from patient record.
- · Obtains insurance authorization.
- · Coordinates with Patient Services Representative (PSR) and Nursing Care Coordinator (NCC) regarding any obstacles associated with obtaining appointment.
- · Participates as a member of the Clinical Support Team and actively engages in strategies to promote the concept of BCHC as a patient centered medical home.
- · Participates in Morning Huddles, using this as an opportunity to address questions or concerns regarding preparedness for patient visits.
- · Assists NCC in establishing self-care management goals with defined patients.
- · Assists NCC in identifying community resources to meet identified needs of patients, including those with advanced care management requirements.
- · Completes activities that help to maintain a clean, organized, and safe work environment that facilitates ease of patient and team member flow and increase patient and team satisfaction.
- · Maintains adequate and appropriate inventory of supplies and forms for use in the clinical area; restocks exam rooms and workstations with supplies, linens, and forms.
- · Completes forms for provider signature.
- · Performs CLIA waived testing after demonstrating competency on each test the facility has a waiver for.
- · Other duties as requested.
