Principal Duties and Responsibilities
- Maintains an active caseload providing interventions as needed, within areas of expertise and within limits of credentials.
- Assess the most urgent needs of patients and working with the health care team, identify ways to assist the patient and coordinate care with community agencies, as appropriate.
- Knowledge of local, state, and federal social service programs.
- Act as a liaison between medical and behavioral care providers to provide continuity of care and quality patient outcome.
- Acquires and provides detailed patient information to establish treatment goals.
- Coordinates the delivery of social services for patients as needed.
- Use professional judgment and discretion to implement approved treatment goals.
- Works with patients on a day-to-day basis implementing treatment goals and follow up to monitor progress.
- Use discretion and judgment to analyze, interpret, make deductions and then decide necessary actions based on the varying facts and circumstances of each individual case.
- Coordinate with the organizations the patients are referred, to confirm patient compliance with recommendations.
- Assists in development, implementation and revision of individual treatment goals.
- Assures that services provided are specified in the patients’ chart and monitors progress toward treatment goals.
- Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
- Consult with external agencies in order to provide clients with support services and resources.
- Maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.
- Provides crisis management for patients as needed.
- Conducts individual and group activities or trainings performed on and off site.
- Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
- Participates in continuing education activities, remaining knowledgeable in area(s) of expertise
- Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision
- Maintains effective, positive and collaborative relationships within and among the Center’s staff, as well as with patients and outside health-care resources.
- Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.
- Ensures and upholds the confidentiality and privacy requirements of all patient records, and manages all daily tasks and activities consistent with HIPAA, state and federal laws and regulations, as well as within BCHC’s policies and procedures regarding confidentiality and security.
- Upholds and ensures compliance with and attention to all BCHC policies and procedures.
- Works to further the mission of BCHC and uphold organizational values.

Knowledge, Skills and Abilities
Key Competencies
- Ability to exercises discretion and judgment to analyze, interprets, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
- Demonstrable knowledge base to successfully function within the position.
- Ability to use practice management and EMR system and standard Office software.
Education and Requirements
- Bachelor’s degree in a related field of study, such as, social work, counseling, human services.
- 3 years experience in a health care or social service setting performing case management with individuals with substance use disorders/mental health disorders.
- CPR or BLCS certification
- Bilingual skills (Spanish) preferred and cultural sensitivity required.
Job Type: Full-time
Pay: $15.02 - $21.36 per hour
