Position Summary
The Community Outreach and Enrollment Specialist will be an integral member of the Berks Community Health Center (BCHC) team. S/he will provide eligibility and enrollment assistance for the Medicaid, CHIP, and Pennie Marketplace health insurance programs to uninsured patients of the health center and community residents. The Community Outreach and Enrollment Specialist will implement in-reach, outreach, and education activities about affordable health insurance options to these individuals and families, and will help them enroll in health insurance.
- Job Description
- · Completes all required federal training to obtain consumer assistance certification, and recertify on an annual basis. Participate in all other required and applicable trainings in order to maintain knowledge and expertise in eligibility, enrollment, and program specifications of the Medicaid, CHIP, and Pennie Marketplace health insurances offered to Berks County residents.
- · Provide application and renewal assistance and facilitate enrollment in health insurance program by providing fair, impartial, and accurate information.
- · Participate in monthly conference and/or supplemental in-person trainings and workshops sponsored by PACHC or other parties, and provide updates to discuss issues, best practices, and modifications or challenges with the online application system for Medicaid, CHIP, and the Pennie Marketplace.
- · Patients will be screened for eligibility for Medicaid, Medicare, CHIP, and Pennie Marketplace health insurance; those that qualify will be assisted with enrollment.
- · Educate applicants about requirements and structure of health insurances programs to prevent loss or lapse of medical coverage.
- · Aid in organizing BCHC classes and events, including, but not limited to New Patient Orientation, Missed Appointment classes, phone call reminders, flyer distribution, donation requests, etc.
- · Represent BCHC in community groups, coalitions and activities working to increase insurance enrollment and access to care for the poor and underserved in the city of Reading and Berks County.
- · Provide culturally appropriate information and assistance in the applicant’s preferred language and or provide limited-English proficiency applicants with oral and written notices of their rights to receive language assistance services and how to obtain such services.
- · Develop strong working relationships with individuals and organizations in Berks County that service the uninsured and medically underserved.
- · Implement outreach activities with community-based organizations, employers, hospitals, schools and other interested parties where education and enrollment can occur.
- · Create and maintain a calendar of events and outreach efforts for the purpose of reporting outreach effectiveness and tracking enrollment activity.
- · Evaluate eligibility and enrollment efforts and prepare reports for BCHC leadership on program results.
- · Advocate for individuals to obtain preventive primary care and other services they may require.
- · Identify service needs in the community and informs BCHC about such needs.
