Position Summary

The Social Determinants of Health (SDOH) Supervisor will provide manage the SDOH navigators and provide support to the patients of the health center and community residents by connecting them to resources to remove social barriers potentially affecting their health (ie: Social Determinants of Health). The Supervisor will be knowledgeable of the PRAPARE tool and educate staff as appropriate. She/he will use the tool to evaluate individual needs and work with the Berks Community Health Center staff to make recommendations for continued care as well as community support services, as needed. This position will collaborate with the health care team to identify social determinants of health including food, transportation, housing, access to medication, utilities assistance, etc. SDOH Supervisor actively supports BCHC in its efforts to maintain designation as a Federally Qualified Health Center (FQHC) and as a Patient Centered Medical Home (PCMH). Participates actively in BCHC’s mission to provide exceptional patient-centered primary and preventive healthcare for all residents of Berks County, regardless of patient status.

Job Description

  • Supervises social determinants of health staff and interns (when available)
  • Addresses barriers to follow through on health care including, but not limited to homelessness/housing instability, financial resources, transportation, childcare, etc. and places referrals to community-based resources as necessary
  • Provides clinical care coordination and care management to patients with social determinant and behavioral health challenges whose assessment tool shows high SDOH needs
  • Provides care coordination and care management services to BCHC patients
  • Coordinates care with internal and external resources to meet identified needs of the patient's care plan and collaborates with providers
  • The SDOH Supervisor is responsible for completing various documents on behalf of BCHC to assist patients with their needs. This includes handling DME requests, Medicare transportation availability, disability documentation, and letters of necessity for housing and insurance. The SDOH Supervisor submits these letters and documents, follows up with agencies to gather necessary information, and reaches out to various resources to ensure the process is completed. Additionally, the SDOH Supervisor helps patients interested in entering nursing homes by initiating the process and collaborates with the Office of Aging and CYS in cases of abuse.
  • Ensures all actions, job performance, personal conduct and communications always represent the organization in a highly professional manner.
  • Ensures and upholds the confidentiality and privacy requirements of all patient records and manages all daily tasks and activities consistent with Health Insurance Portability and Accountability Act (HIPAA), state and federal laws and regulations, as well as within BCHC’s policies and procedures regarding confidentiality and security.
  • Creates and maintains relationships with community-based resources to refer to.
  • Monitors and evaluates effectiveness of plan and modifies plan as needed
  • Provides concise and thorough documentation of assessment and activities.
  • Works to further the mission of BCHC and uphold organizational values.

  • Other Responsibilities

Key Competencies

  • Bi-Lingual (Spanish) is preferred.
  • Able to strongly embrace and personify the mission of BCHC.
  • Have understanding and knowledge of uninsured, underserved, and underrepresented population within Reading and Berks County.
  • Good communication skills: able to listen well, use language appropriately, and be able to communicate well both verbally and in writing.

Education and Requirements

  • Bachelor’s degree in Social Work required
  • A minimum of 2 years of work experience in health, social or community services, or comparable experience preferred.
  • Microsoft Office computer skills, required
  • Local travel is required for the position. A valid PA driver’s license and automobile insurance are also required.

Job Type: Full-time

Pay: $60,000.00 - $65,000.00 per year
