As a Re-Entry Clinical Care Coordinator, you will provide advanced level care coordination to incarcerated individuals transitioning from BCC’s Substance Use Disorder Program at Berks County Jail System and advanced level administrative support to the program.
You will…
- Provide screening and evaluations of social determinants of health.
- Provide a warm handoff – linking resources and facilitating transition of care between service providers.
- Perform data collection and reporting and surveying.
- Oversee the coordination of care across programs.
- Participating in Continuous Quality Improvement activities and huddles.
- Provide coordination with criminal justice system (probation and parole, BCJS, etc.) department staff and scheduling to ensure patient access to care.
- Work closely with your Direct Supervisor and collaboratively with other Department Supervisors within the agency.
Requirements
To qualify for the position, you will need…
- An associate degree
- To have one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity
- To be able to effectively facilitate face to face and virtual services in the jail setting and in the community.
- To be able to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
Preferred candidates will …
- Be Bilingual: English/Spanish
- Have Knowledge of or experience with trauma-informed approaches.
Physical Requirements:
- SEEING: Must be able to see well enough to read reports and use computer.
- HEARING: Must be able to communicate with patients, vendors, and coworkers.
- MOBILITY: Must have mobility to move about the agency.
- LIFTING/PULLING/PUSHING: Must be able to lift up to 5 pounds.
- OTHER: Must be able to write, type, and use office technology proficiently.

- Receive clearance from BCJS.
- Receipt of results of a satisfactory drug and alcohol screening test.
- Receipt of the originals of the following background checks: PA State Police Criminal History, PA State Child Abuse, and DPW FBI Criminal History. These clearances must be valid within the 60 months prior to employment. A conviction is not a bar to employment; however, a review will be conducted to make a determination of employment should a conviction be present.
- Receipt of original documentation and verification of relevant degrees, diplomas, certificates, or licensure.
- Confirmation that you are not on the OIG Federal Healthcare Program Exclusion from Participation list. Should you appear on this list, Berks Counseling Center is prohibited from employing you.
