General Summary
You will be responsible for completing a comprehensive clinical assessment on potential clients seeking admission to Berks Counseling Center as part of our Open Admission process. This will include completion of supporting documentation, orienting the client to services, and setting up the electronic health record.
Essential Job Functions
- Conduct face to face clinical behavioral health assessments on persons seeking services at Berks Counseling Center as part of BCC’s multidisciplinary admissions team.
- Determine an appropriate level of care for each client based on established guidelines.
- Assign client to an appropriate clinician based on his/her needs.
- Utilize collaborative documentation to complete an intake note and comprehensive clinical assessment that meets compliance standards, medical necessity guidelines, and agency standards as to timeliness and accuracy of documentation.
- Orient the client to agency policies and procedures and secure necessary consents and releases.
- Responsible for opening and inputting initial clinical data into the client electronic health record.
- Work collaboratively with and communicate and meet regularly with other members of BCC’s Admissions Team.
- Work collaboratively and cooperatively with outside referral sources.
- Regularly prepare information and reports, as directed, on admissions activities that support decision making and program planning and development.
- Identify and respond to emergency clinical needs and crises as they arise.
- Utilize individual and group supervision to review clinical cases and seek feedback and direction as needed.
- Navigate and utilize electronic health records using technology, including computers.
- Develop an understanding of trauma and how it impacts others.
- Incorporate trauma informed practices into job duties and responsibilities, including interactions and communications with others.
- Be an active member of BCC’s culture of caring through positive and engaging social interactions.
- Perform all other related duties and responsibilities as assigned.
Requirements
Minimum Qualifications
- Active PA Licensure (LPC, LCSW, LMFT) with counseling experience in mental health and/or drug and alcohol required.
- Knowledge of or experience with trauma-informed approaches preferred.
- At least one (1) year documented work experience in a behavioral health setting required.

- Have at least 1 year training or experience relevant to working in a medical office with mental health or substance use treatment preferred.
- Be Bilingual: English/Spanish preferred.
- Have Knowledge of or experience with trauma-informed approaches preferred.
Physical Requirements
SEEING: Must be able to see well enough to read reports and use computer.
HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers.
MOBILITY: Must have mobility to move about the agency.
LIFTING/PULLING/PUSHING: Must be able to lift up to 5 pounds.
OTHER: Must be able to write, type and use technology.

- Receipt of original documentation and verification of relevant degrees, diplomas, certificates, or licensure.
